What to Bring to Your Child's First Paediatric Appointment
Child's Birth Record and Hospital Summary
Bring the hospital discharge summary from delivery — this contains important information about gestational age, birth weight, delivery type, any complications, NICU stay, and early screening results (newborn hearing screen, metabolic screening).
Previous Vaccination Records
Bring any vaccination cards or records from previous vaccines given at the hospital or another clinic. This allows Dr. Sushma to assess vaccination status and plan the catch-up or next schedule correctly.
Your Child's Growth Records
Bring any previous weight, height, and head circumference measurements — from previous clinic visits, weighing at the hospital, or the Road to Health chart. Tracking growth over time is more informative than a single measurement.
Current Medicines and Vitamins
Bring all medicines, vitamin drops, supplements, and traditional medicines your child is currently taking — including dose and frequency. Include any medicines given for the current illness.
List of Your Concerns — Written Down

For Newborns — Feeding Information

History Taking — Your Child's Story
Dr. Sushma will ask about the current concern, pregnancy and birth history, feeding history, developmental milestones (smiling, sitting, walking, talking), vaccination history, family history of health conditions, and social and school history for older children. This is the most important part of the visit.
Full Physical Examination
A complete head-to-toe examination including: weight, height and head circumference on WHO growth charts; skin (rashes, jaundice); head (fontanelle in infants); eyes and ears; throat and lymph nodes; heart and lungs; abdomen; genitalia (in neonates); hips; and neurological assessment (tone, reflexes, developmental milestones).
Developmental Assessment
Age-appropriate developmental screening — gross motor (sitting, walking, running), fine motor (grasping, drawing), language (sounds, words, sentences), social skills (eye contact, play, interaction). Concerns are documented and formal assessment arranged if needed.
Diagnosis and Management Plan
