Dystonia Treatment in Hyderabad — Involuntary Muscle Contractions & Abnormal Posture
Dystonia causes involuntary, sustained muscle contractions that result in abnormal postures or repetitive twisting movements. It is under-diagnosed but highly treatable by an experienced movement disorder neurologist.
Dystonia is a neurological movement disorder characterised by sustained or intermittent muscle contractions causing abnormal, often repetitive movements and postures. It affects people of all ages and can involve any body part. Dr. Anand Karnam at Sri Anand Child and Neuro Center has experience in diagnosing and managing various forms of dystonia — from focal forms (affecting one body part) to generalised dystonia — using a combination of medication and referral for botulinum toxin therapy where appropriate.
Types of Dystonia — Symptoms & Affected Areas
Cervical Dystonia (Torticollis)
The most common focal dystonia — involuntary contraction of neck muscles causing the head to rotate, tilt, or pull forward/backward. Causes sustained pain and abnormal head posture. Often mistaken for a 'stiff neck'. Treatment with botulinum toxin injections (every 3 months) gives excellent results.
Writer's Cramp (Task-Specific Dystonia)
Involuntary cramping or abnormal posturing of the hand and forearm specifically during writing. Common in professionals who write extensively. Physiotherapy (sensory tricks, task-specific retraining) and sometimes botulinum toxin injections are effective.
Blepharospasm — Eyelid Dystonia
Involuntary, forceful, repeated eye closure. Can be socially disabling and in severe cases prevents normal vision. Botulinum toxin injections into the orbicularis oculi muscle provide excellent relief. Requires referral to a specialist injector.
Generalised Dystonia
Involves multiple body regions — often begins in childhood or adolescence. Can be caused by genetic mutations (DYT1, DYT6), Wilson's disease, or neurological injury. Requires detailed investigation (genetic testing, copper studies, MRI) and specialist management.
Secondary Dystonia
Dystonia can be caused by brain injury (stroke, hypoxia), medication side effects (dopamine blockers, metoclopramide, antipsychotics), or Wilson's disease (treatable copper metabolism disorder in young people). Identifying and treating the underlying cause is crucial.
Childhood Dystonia
When a child develops abnormal posturing or twisting movements, it requires urgent neurological evaluation. Causes include genetic dystonia, metabolic disorders (Wilson's disease, glutaric aciduria), and cerebral palsy. Early diagnosis changes long-term outcomes significantly.
Dystonia Treatment Options
Oral Medications
Trihexyphenidyl (anticholinergic), baclofen (muscle relaxant), and clonazepam are first-line oral medications for generalised dystonia, especially in children. Levodopa is dramatically effective in Dopa-responsive dystonia (DRD) — a treatable genetic form that mimics cerebral palsy.
Botulinum Toxin Injections (Referral)
The gold standard for focal dystonias (cervical dystonia, blepharospasm, writer's cramp). Injections into affected muscles provide 3 months of significant relief. Dr. Anand Karnam advises on appropriate cases and coordinates referral to centres providing this treatment in Hyderabad.
Physiotherapy & Sensory Tricks
Physiotherapy helps with stretching, strengthening, and — importantly — sensory tricks (tactile stimulation of the affected area can temporarily reduce dystonia). Coordinated with Dr. Harisha (MPT) at Sri Anand CNC.
Deep Brain Stimulation (DBS) — Referral for Severe Cases
For severe generalised dystonia (particularly DYT1 genetic dystonia) not responding to medication, Deep Brain Stimulation — electrode implantation in the globus pallidus — produces remarkable improvement. Dr. Anand Karnam identifies candidates and facilitates referral to DBS centres.
Wilson's Disease Treatment
When dystonia is caused by Wilson's disease (a treatable copper metabolism disorder), chelation therapy (D-penicillamine, trientine) or zinc supplementation stops progression and often reverses symptoms. This is why all young patients with dystonia require copper screening.
