Balance Problems and Unsteady Walking — Causes and Treatment in Hyderabad

Balance depends on three systems working together: the inner ear (vestibular system), vision, and joint position sense (proprioception) — all coordinated by the cerebellum and brain. Problems in any of these systems cause imbalance and unsteady walking. Possible Causes of This Symptom

Vestibular Disorders (Inner Ear)

Conditions like BPPV, vestibular neuritis, or Menière's disease cause imbalance with dizziness and spinning. The inner ear sends confused signals to the brain. Usually treatable with specific manoeuvres or medication.

Cerebellar Disease

The cerebellum coordinates movement. Disease here (from stroke, multiple sclerosis, alcohol, or hereditary conditions) causes characteristic wide-based, staggering gait — like walking while drunk. MRI brain is diagnostic.

Parkinson's Disease

Balance problems in Parkinson's are due to impaired postural reflexes — patients fall easily because they cannot automatically correct their balance. Accompanied by slowness, tremor, and rigidity.

Peripheral Neuropathy

Damage to the sensory nerves in the feet (from diabetes, B12 deficiency) causes loss of position sense — the person cannot feel where their feet are relative to the ground. Balance is especially poor in the dark. NCS confirms the diagnosis.

Cervical Myelopathy (Spinal Cord Compression)

Arthritic changes in the neck compressing the spinal cord cause unsteady walking, difficulty with fine finger movements, and electric-shock sensations on bending the neck. MRI cervical spine is diagnostic.

Tandem walking (heel-to-toe), Romberg test, finger-nose test, and heel-shin test identify the type of balance problem and point to inner ear, cerebellar, spinal cord, or sensory causes.

MRI Brain and Cervical Spine

Essential when cerebellar or spinal cord disease is suspected. Identifies stroke, MS plaques, cerebellar atrophy, disc disease, or cord compression.

Nerve Conduction Study (NCS)

Confirms peripheral neuropathy contributing to sensory ataxia and identifies the type and severity of nerve damage.
