In 2023, the World Health Organization issued an unambiguous statement: there is no safe level of alcohol consumption when it comes to cancer risk. Every drink increases cancer risk. This does not mean one drink will cause cancer — but it means there is no threshold below which alcohol is completely safe.
The 7 Cancers Causally Linked to Alcohol
- Mouth cancer: Alcohol damages the oral mucosa directly and acts as a solvent for tobacco carcinogens (the combination of smoking and drinking multiplies risk far beyond either alone)
- Throat/pharyngeal cancer: Same mechanism as mouth cancer
- Oesophageal cancer: Acetaldehyde — the primary carcinogenic breakdown product of alcohol — is produced in high concentration in the oesophagus
- Laryngeal cancer: Direct tissue damage and acetaldehyde exposure
- Liver cancer: Alcohol causes cirrhosis → liver cancer in a well-established progression
- Colorectal cancer: Alcohol alters gut microbiome, increases bowel mucosal permeability, and directly causes DNA damage in bowel cells
- Breast cancer: Alcohol elevates circulating oestrogen levels — even modest drinking increases breast cancer risk by 7–10% per drink per day
The Acetaldehyde Mechanism
When alcohol (ethanol) is metabolised in the body, the first breakdown product is acetaldehyde — a compound that directly damages DNA in cells. The ALDH2 enzyme converts acetaldehyde to the harmless acetate. Many East Asians (and some South Asians) have a less active ALDH2 variant — they accumulate more acetaldehyde and have significantly higher cancer risk from the same alcohol intake.
The Indian Context
In India, alcohol consumption has risen significantly over the past two decades — particularly among urban young adults and women. The social normalisation of "social drinking" or "one drink at a party" needs to be reconsidered in light of this evidence. This does not mean prohibition — it means making an informed choice. The risk from one drink is small but real; the risk from regular daily drinking is substantial.
