The fear that comes with a persistent headache — "could it be something serious, like a brain tumour?" — is extremely common. The reassuring truth is that the vast majority of headaches are NOT due to brain tumours. But there are specific warning signs that should prompt urgent evaluation.
The Reassuring Majority
The most common types of headache — tension-type headaches and migraines — are not caused by brain tumours and are not dangerous to your brain. They can be thoroughly debilitating, but they are not a sign of something sinister. A neurologist can diagnose these conditions confidently based on history alone in most cases.
Warning Signs That Need Urgent Evaluation
These are the "red flags" that should prompt you to see a neurologist promptly — and in some cases, go to an emergency department:
- Thunderclap headache: A sudden, severe headache that reaches maximum intensity within seconds — the "worst headache of my life." This needs emergency evaluation to rule out subarachnoid haemorrhage (bleeding around the brain).
- Progressive headache: A headache that gets steadily worse over weeks or months
- Headache that wakes you from sleep
- Headache significantly worsened by coughing, sneezing, or bending down (raised intracranial pressure)
- New headache in someone over 50 who never had headaches before
- Headache accompanied by fever and neck stiffness (meningitis)
- Headache with neurological symptoms: weakness, speech changes, vision changes, coordination problems, seizures
- Headache in a cancer patient — could indicate spread to the brain
- Headache in an immunocompromised person (HIV, on steroids)
Brain Tumour Symptoms Beyond Headache
Brain tumours cause symptoms depending on their location. Common symptoms include:
- New seizures (fits) in an adult who never had them before
- Gradually worsening weakness on one side of the body
- Speech difficulties — slurring or inability to find words
- Vision changes — blurring, double vision, loss of peripheral vision
- Personality or behaviour changes
- Difficulty with balance or coordination
Diagnosis and Next Steps
If a brain tumour is suspected, MRI brain (with contrast) is the investigation of choice. Most headaches do not require MRI — a neurologist will assess whether it is needed based on your history and examination.
