India has more than 100 million people with diabetes — the highest absolute number in the world. While much attention is paid to blood sugar control, the most important consequence of poorly controlled diabetes is the damage it causes to small blood vessels (microvascular disease) — affecting the nerves, kidneys, and eyes.
Why Do Diabetes Complications Develop?
Persistently elevated blood glucose damages small blood vessels through multiple mechanisms: advanced glycation end-products (AGEs), oxidative stress, inflammation, and polyol pathway activation. The result is progressive damage to:
- Peripheral nerves → diabetic neuropathy
- Kidney glomeruli → diabetic nephropathy
- Retinal vessels → diabetic retinopathy
- Large vessels → heart attack, stroke (macrovascular)
The critical point: complications begin developing 7–10 years before diabetes is diagnosed in many patients. Screening at diagnosis is essential.
Diabetic Neuropathy — The Most Common Complication
Affects 50% of diabetics within 10 years of diagnosis. Types:
- Distal symmetric polyneuropathy: Most common. Numbness, tingling, and burning in the feet and hands — 'glove and stocking' pattern. Starts in the feet and moves upward. Loss of protective sensation leads to unnoticed foot injuries and diabetic foot ulcers
- Autonomic neuropathy: Affects heart rate, blood pressure, digestion, bladder, and sexual function. Causes: postural hypotension (dizziness on standing), gastroparesis (nausea, early satiety), constipation/diarrhoea, bladder retention, erectile dysfunction
- Mononeuropathy: Sudden single nerve palsy — e.g., foot drop, wrist drop, third nerve palsy causing double vision

Diabetic Kidney Disease (Nephropathy)
- Early: microalbuminuria — detected on urine test (urine albumin:creatinine ratio)

Screening: Annual urine albumin:creatinine ratio + serum creatinine (eGFR) from diagnosis. ACE inhibitors or ARBs protect the kidney.
Diabetic Retinopathy — Preventable Blindness
- Affects 34% of diabetics
- The leading cause of blindness in working-age adults in India
- Often has no symptoms until advanced
Screening: Annual dilated fundoscopy (retinal examination) by an ophthalmologist from the time of diagnosis. Laser treatment prevents blindness in most cases when detected early.
Diabetes and Stroke Risk
Diabetics have 2–4× the risk of stroke compared to non-diabetics. Neurological screening in diabetes includes:
- Blood pressure control (target <130/80 in diabetics)
- Cholesterol management (LDL <70 mg/dL in high-risk diabetics)
- Aspirin for high-risk patients
- Smoking cessation
