Everyone forgets things as they age. But there is a critical difference between normal ageing memory and the early signs of dementia. Dr. Anand Karnam explains exactly where the line is — with practical examples.

One of the most common questions a neurologist receives from middle-aged and older patients: "I keep forgetting things — is this the beginning of dementia?" In most cases, the answer is no. Normal ageing does affect memory — but in specific and predictable ways that are fundamentally different from the memory failure of dementia. Understanding the distinction prevents unnecessary anxiety while ensuring that true warning signs are not dismissed as "just ageing."

Normal Age-Related Memory Changes

Processing speed slows — it takes longer to recall a word or name but it eventually comes. Difficulty with multi-tasking and divided attention — harder to do two cognitively demanding tasks simultaneously. Slower learning of new information — but once learned, it is retained. Names are harder to recall on demand but recognised when heard. Keys are occasionally misplaced — but can be found by retracing steps. Appointments may be forgotten — but remembering them when reminded with a cue. The person is aware of their forgetting and concerned about it.

Dementia Warning Signs — Not Normal

Forgetting entire conversations that occurred recently — not just details but the entire conversation. Asking the same question multiple times within the same hour without remembering they asked. Getting lost in familiar places — a route they have driven for years. Forgetting how to do previously automatic tasks — preparing familiar recipes, operating familiar appliances. Personality change — becoming paranoid, suspicious, or uncharacteristically aggressive. Difficulty managing finances — missing bills, making unusual purchases. Not recognising family members they see regularly. Neglecting personal hygiene. The person is often unaware of their memory problems (lack of insight is an important sign).

The Key Distinction

In normal ageing: forgetting the name but remembering it later; misplacing keys but finding them; aware of the problem.
