GP or Specialist: Who Should You See First? A Hyderabad Patient's Guide

One of the most common questions I hear from patients: "Should I have gone to a specialist directly? Or was it right to start with a GP?" It's an excellent question — and the answer can save you significant time, money, and unnecessary tests.

What Is a General Physician (GP)?

A general physician (also called a family doctor, GP, or internal medicine doctor) is a qualified specialist in adult health — trained to diagnose and treat a wide range of conditions across all organ systems. In India, GPs have postgraduate qualifications such as MD General Medicine or DNB General Medicine after MBBS.

A GP is your first point of contact for most new health concerns. They manage many conditions completely and refer you to the right specialist when necessary — with proper documentation and context that saves time at the specialist's end too.

When Should You See a GP First?

For most new or ongoing health concerns, starting with a GP is the right move. See a general physician first for:

  • Fever, infections, or flu-like symptoms
  • Diabetes — blood sugar control, medication review, HbA1c testing
  • High blood pressure — diagnosis and ongoing management
  • Thyroid conditions (hypothyroidism, hyperthyroidism)
  • Fatigue, unexplained weight gain or loss, weakness
  • Stomach and digestive problems — acidity, indigestion, diarrhoea
  • Annual health checkup and routine blood tests
  • Vitamin deficiencies — B12, Vitamin D, iron-deficiency anaemia
  • Cough, cold, and mild respiratory infections
  • Headache and body pain that has been occurring for some time

A good GP can manage 80% of adult health conditions effectively. For the remaining 20% that need specialist involvement, they will refer you — often with test results already in hand, so the specialist can act immediately.

When to Go Directly to a Specialist?

Some situations genuinely warrant a direct specialist visit without going to a GP first:

  • Neurologist: Sudden weakness or numbness on one side of the body, sudden slurred speech, a first-ever seizure (fit), recurring severe headaches with neurological symptoms, or significant memory problems
  • Paediatrician: Any health concern in a child under 18 — especially newborn care, developmental concerns, or vaccinations
  • Cardiologist: Chest pain, palpitations, or known heart disease
  • Orthopaedic surgeon: Fractures, joint injuries, or severe arthritis

The Hidden Cost of Bypassing the GP

When patients go directly to a specialist for every issue — fever, fatigue, cough, blood pressure — they often end up:

  • Waiting weeks for a specialist appointment
  • Paying specialist consultation fees for conditions a GP can treat on the spot
  • Undergoing tests ordered by the specialist, some of which a GP would have done first anyway
  • Receiving fragmented care with no one keeping track of the full picture

The GP is the person who keeps the overview of your health — your blood pressure trend, your medication history, your diabetes control over years. A specialist who sees you once cannot replicate that continuity.
