A stroke kills or disables one in four people over the age of 25. Yet the World Health Organization estimates that 80% of strokes could be prevented with simple, consistent lifestyle and medical interventions. Here are the 8 most impactful steps.
1. Control Your Blood Pressure
High blood pressure is the single biggest modifiable stroke risk factor — responsible for over 50% of strokes. Target: below 130/80 mmHg. Measure your BP monthly at home. If you are on antihypertensives, take them daily without skipping. Even a 10 mmHg reduction in systolic BP cuts stroke risk by 27%.
2. Know and Treat Atrial Fibrillation (AF)
Atrial fibrillation — an irregular heart rhythm — causes 20–25% of all strokes. It often has no symptoms. Ask your doctor to check your pulse at your next appointment. If diagnosed with AF, blood thinners (anticoagulants) reduce stroke risk by 60–70%. Do not skip these medicines.
3. Stop Smoking Immediately
Smoking doubles stroke risk. Within 5 years of stopping, stroke risk returns to near-normal. No amount of smoking is safe for stroke risk. This is the single most impactful lifestyle change a smoker can make for their brain.
4. Manage Diabetes

5. Control Cholesterol
High LDL cholesterol accelerates atherosclerosis (plaque in arteries), narrowing vessels that supply the brain. If you have had a stroke or TIA, a statin is essential. Target LDL below 70 mg/dL for very high-risk patients.
6. Exercise Regularly
30 minutes of moderate aerobic exercise (brisk walking, cycling, swimming) 5 days a week reduces stroke risk by 25–30%. Exercise lowers BP, improves cholesterol, reduces body weight, and has direct neuroprotective effects.
7. Eat a Heart-Healthy Diet
The Mediterranean diet (vegetables, fruits, whole grains, legumes, fish, olive oil — low in red meat and processed foods) is the most evidence-based diet for stroke prevention. Reduce salt intake to under 5g/day. Avoid processed and ultra-processed foods.
8. Act Immediately on TIA Warning Signs
A TIA (transient ischaemic attack or "mini-stroke") causes temporary stroke symptoms that resolve completely within 24 hours. It is a medical emergency — 10% of patients have a full stroke within 48 hours of a TIA. If you or someone near you has sudden face drooping, arm weakness, or speech difficulty — even briefly — go to hospital immediately. Do not wait to see if it passes.
