What the MRI Shows

A cross-sectional MRI of the thigh shows muscle tissue (dark on MRI), fat (bright), and bone. In a sedentary 40-year-old, the cross-section often shows extensive fat infiltration within and around the muscle bundles — what looks like a well-marbled steak. In an active 74-year-old, the cross-section shows dense, clean muscle with minimal fat infiltration — resembling the thigh of a much younger person.

This is not genetics. It is lifestyle — accumulated over decades.

Sarcopenia — Muscle Loss With Age

After age 30, humans lose 3–5% of muscle mass per decade without targeted resistance exercise. By 60, a sedentary person may have lost 30% of their peak muscle mass. This is sarcopenia — and it has consequences far beyond aesthetics:

  • Reduced metabolic rate → weight gain and diabetes risk
  • Increased fall risk → fractures in older age
  • Reduced grip strength → an independent predictor of cardiovascular mortality
  • Reduced insulin sensitivity → type 2 diabetes
  • Cognitive decline → muscle produces BDNF (brain fertiliser) during exercise

The Good News: It Is Reversible

Studies in people in their 70s and 80s show significant muscle mass gain with resistance training — the muscle fibres respond to stimulus at any age. It is never too late to start. The recommendation from exercise science: 2–3 sessions of resistance training per week (bodyweight, bands, or weights) combined with adequate protein intake (1.2–1.6 g/kg/day).
