One of the most common points of confusion for patients with brain, spine, or nerve conditions: should I see a neurologist or a neurosurgeon? The answer matters — because starting with the wrong specialist can delay correct treatment by months.
The Simple Distinction
- Neurologist: A physician who diagnoses and treats neurological conditions with medication, lifestyle intervention, physiotherapy, and non-surgical procedures
- Neurosurgeon: A surgeon who performs operations on the brain, spine, and peripheral nerves
Both are trained in neurology — but a neurosurgeon's additional training is in surgical technique. A neurologist does not perform surgery; a neurosurgeon primarily operates.
What Does a Neurologist Treat?
Neurologists manage the vast majority of neurological conditions — without surgery:
- Headache and migraine
- Epilepsy and seizures
- Stroke (acute treatment and prevention)
- Parkinson's disease and movement disorders

- Peripheral neuropathy (diabetic, B12, autoimmune)

What Does a Neurosurgeon Treat?
Neurosurgeons operate for conditions where surgery is the right treatment:
- Brain tumours requiring resection
- Ruptured cerebral aneurysm
- Acute subdural or extradural haematoma
- Spinal disc surgery (microdiscectomy) for severe or refractory sciatica
- Cervical spine surgery (ACDF) for myelopathy
- Spinal fusion for instability
- Deep Brain Stimulation (DBS) for Parkinson's and dystonia
- Epilepsy surgery
- Carpal tunnel release (when conservative treatment fails)
- Lumbar canal stenosis surgery
The Most Important Rule: Start with a Neurologist
For almost all non-emergency neurological symptoms, the correct first specialist is a neurologist. Here's why:
- Most conditions don't need surgery: Over 95% of neurological conditions presenting to a clinic are managed non-surgically. Surgery is the last resort — not the first step
- Accurate diagnosis first: A neurologist diagnoses the exact condition before deciding whether surgery is even indicated. Many patients referred for 'brain surgery' actually have conditions like migraine, anxiety-related symptoms, or medication-treatable epilepsy
- Neurologist manages pre- and post-op care: Even when surgery is needed, neurological care before and after surgery is essential — the neurosurgeon operates, the neurologist manages everything else
When to Go Directly to a Neurosurgeon
Go directly to a neurosurgeon (emergency) for:
- Head injury with loss of consciousness or worsening headache
- Sudden severe ('thunderclap') headache — worst headache of your life
- Suspected spinal cord compression with progressive paralysis
- Suspected brain abscess or haematoma after trauma
These are neurosurgical emergencies — go to the nearest neurosurgery-capable hospital immediately.
