Post-acute sequelae of COVID-19 (PASC) — commonly called long COVID — affects an estimated 10–30% of people who had COVID-19, regardless of severity of the initial infection. Among the most common and disabling symptoms are neurological: persistent fatigue, cognitive impairment (brain fog), headaches, sleep disturbances, anxiety, and autonomic dysfunction. These can last months to years after the acute infection has resolved.
What Is Post-COVID Brain Fog?
Patients describe it as: "I can't concentrate for more than 10 minutes"; "I forget words mid-sentence — words I've used my whole life"; "I feel like I'm thinking through cotton wool"; "I can't do my job — I used to handle complex tasks easily." These are not just descriptions of tiredness. Objective neuropsychological testing in long COVID patients demonstrates measurable deficits in processing speed, working memory, attention, and executive function.
The Neurological Mechanisms
Neuroinflammation: SARS-CoV-2 activates microglial cells (the brain's immune cells), triggering chronic neuroinflammation that disrupts neural signalling. PET scan studies show persistent microglial activation in long COVID patients months after infection.
Cerebrovascular damage: COVID-19 causes endothelial (blood vessel lining) damage and micro-clotting throughout the body, including in cerebral small vessels. Reduced cerebral blood flow and small vessel injury impair cognitive function.
Autonomic nervous system dysfunction: Post-COVID POTS (postural orthostatic tachycardia syndrome) — heart rate surging on standing — dramatically impairs brain perfusion and produces cognitive symptoms.
Sleep disruption: Almost universal in long COVID. Sleep is critical for cognitive function and memory consolidation — fragmented sleep perpetuates brain fog.
What Helps
- Pacing — avoiding post-exertional malaise by not pushing through fatigue; graduated return to activity
- Sleep hygiene and treatment of specific sleep disorders (polysomnography may identify sleep apnoea)
- POTS management: increased salt and fluid intake, compression stockings, beta-blockers if required
- Cognitive rehabilitation: structured brain exercises with an occupational therapist or neuropsychologist
- Anti-inflammatory strategies: omega-3 supplementation, Mediterranean diet
- Treatment of comorbid anxiety and depression (extremely common in long COVID) — affects cognitive function profoundly
