Back pain is one of the most common complaints seen in any neurology or general practice clinic. But not all back pain is the same — and knowing whether your back pain involves nerve compression (sciatica) or is purely muscular or skeletal changes your treatment significantly.

What Is Sciatica?

Sciatica is not a diagnosis by itself — it is a symptom. It refers to pain caused by irritation or compression of the sciatic nerve or its nerve roots (L4, L5, S1, S2, S3). The sciatic nerve is the largest nerve in the body — it starts in the lower back, passes through the buttock, and runs down the back of the leg to the foot.

True sciatica almost always causes pain that radiates below the knee — often all the way to the foot. This radiation pattern is the key feature.

What Causes Sciatica?

The most common cause (by far) is a lumbar disc prolapse — when the soft inner material of an intervertebral disc pushes outward and presses on a nerve root. The L4-L5 and L5-S1 disc levels are most commonly affected.

Other causes include:

  • Lumbar canal stenosis — narrowing of the spinal canal, common in adults over 50
  • Piriformis syndrome — compression of the sciatic nerve by the piriformis muscle in the buttock
  • Spondylolisthesis — slipping of one vertebra over another
  • Tumour or infection — rare but must be excluded (especially with unexplained weight loss or fever)

  • Foot drop (inability to lift the foot while walking)
  • Numbness around the groin, inner thighs, or saddle area
  • Loss of bladder or bowel control

Diagnosis — What Tests Are Done?
