Among the modifiable risk factors for stroke, smoking is one of the most powerful and most underappreciated. The relative risk of ischaemic stroke in smokers is 2–4 times that of non-smokers — comparable to the risk added by hypertension. In young adults (under 45), smoking is the single most common modifiable cause of stroke. Despite this, smoking cessation counselling remains underutilised in India, where tobacco use — including bidis, gutka, and khaini — is endemic.
How Smoking Damages the Brain
Accelerated atherosclerosis: Tobacco smoke compounds — nicotine, carbon monoxide, and thousands of combustion chemicals — damage the endothelium (inner lining) of blood vessels. This triggers inflammatory plaques in the carotid arteries (the main arteries supplying the brain) that narrow and eventually rupture, causing embolic stroke.
Increased blood clotting: Smoking increases platelet aggregation (stickiness) and fibrinogen levels — both promoting clot formation. Combined with narrowed arteries, this dramatically increases thrombotic stroke risk.
Elevated blood pressure: Each cigarette causes an acute spike in blood pressure of 5–10 mmHg lasting 30 minutes. Chronic smokers have persistently elevated BP — a direct stroke risk factor.
Reduced oxygen delivery: Carbon monoxide in smoke binds haemoglobin 200 times more avidly than oxygen, reducing blood oxygen-carrying capacity by 5–15%. Chronic mild brain hypoxia accelerates neurodegenerative changes.
Cerebral small vessel disease: Smoking is strongly associated with lacunar infarcts — small vessel strokes deep in the brain — and with white matter hyperintensities on MRI that indicate diffuse brain damage.
The Good News: Quitting Works Rapidly
- Within 20 minutes: BP and heart rate normalise
- Within 12 hours: Carbon monoxide levels normalise; blood oxygen improves
- Within 1 year: Excess stroke risk reduced by 50%
- Within 5 years: Stroke risk approaches that of a non-smoker
It is never too late to quit — even in someone who has smoked for 40 years, quitting at 60 significantly reduces stroke risk. Effective cessation aids available: nicotine replacement therapy (patches, gum), varenicline (Champix), and bupropion. Combining medication with behavioural counselling doubles the success rate.
