Snoring: When Is It Just Noise and When Is It Dangerous?

50% of adults snore. Most snoring is benign — but snoring with breathing pauses, morning headaches, or daytime sleepiness signals obstructive sleep apnea. Dr. Anand Karnam explains the difference and when to seek help.

Snoring — the sound produced by vibration of the soft tissues of the upper airway (soft palate, uvula, tonsillar pillars) during sleep — is one of the most common sleep-related complaints globally, affecting approximately 40–50% of adults. The key clinical question is not "does this person snore?" but "does this snoring indicate obstructive sleep apnea?" Primary snoring (without apnea) is generally benign; OSA is a serious medical condition with significant cardiovascular and neurological consequences.

Primary Snoring vs OSA

Primary snoring: Regular, habitual snoring without respiratory pauses, oxygen desaturations, or EEG arousals. The person sleeps through the night and wakes refreshed. No cardiovascular or neurological consequences. No treatment beyond positional advice and weight loss if applicable. Obstructive sleep apnea (OSA): Snoring punctuated by silent pauses (the partner notices breathing has stopped) followed by gasping or snorting arousals. Restless sleep; multiple awakenings; nocturia; morning headaches; daytime sleepiness. Requires polysomnography and CPAP treatment.

Risk Factors for OSA in a Snorer

Male sex; age above 40; obesity (BMI above 25 — neck fat deposits compress the airway); neck circumference above 40cm in women, 43cm in men; retrognathia (recessed jaw); large tonsils (especially in children); nasal obstruction; alcohol use (relaxes pharyngeal muscles).
