In Hyderabad's neurology clinics, an increasing proportion of stroke patients are in their 20s, 30s, and 40s — working professionals, parents of young children, students — whose lives are upended by a brain event that "only happens to old people." Why Are Young Adults Having Strokes?
The causes of stroke in young adults differ from older patients:
- Hypertension discovered too late: Many young adults have undiagnosed high BP. With no symptoms for years, the first sign is a stroke at 35
- Patent Foramen Ovale (PFO): A hole between heart chambers present from birth — allows clots to bypass the lungs and travel directly to the brain
- Carotid artery dissection: A tear in the carotid artery wall — caused by neck manipulation, sports injuries, or even violent coughing
- Antiphospholipid syndrome: A blood-clotting disorder — especially in young women
- Drug use: Stimulant drugs cause severe vasospasm and stroke
- Migraine with aura: Significantly increases stroke risk in young women who smoke or take oral contraceptives
- Obesity and diabetes in younger age groups: Metabolic risk factors now presenting a decade earlier than previous generations
Why Children Can Have Strokes Too
Paediatric stroke — stroke in children — is rarer but real. Causes include congenital heart disease, sickle cell disease, arteriovenous malformations (AVMs), and infections. A child with sudden facial droop, arm weakness, or speech difficulty must be taken to a stroke-capable hospital immediately — the same emergency rules apply.

What Makes Young Adult Stroke Different
Young stroke patients typically survive — but many are left with disability at the most productive period of their lives. They face 40+ more years living with paralysis, aphasia, or cognitive impairment. Secondary prevention, rehabilitation, and identifying the underlying cause (to prevent recurrence) are critical. Many young stroke patients in Hyderabad never receive a full cardiac and haematological workup — missing the underlying cause that will cause the next stroke.
