In Hyderabad's winter months (December–February), many people notice their feet and lower legs feeling colder and sometimes numb. For most people, this is simply vasoconstriction — blood vessels narrowing in response to cold to preserve core body temperature, a completely normal physiological response. But in some people, this cold-triggered leg numbness is a signal of an underlying condition that needs medical attention.
Normal Cold Response vs Concerning Symptoms
Normal: Mild coldness and reduced sensation in the feet during cold weather, which resolves completely with warming. No pain, no colour changes beyond mild pallor.
Concerning symptoms requiring evaluation:
- Numbness present even at room temperature — not related to cold
- Burning or tingling in the feet and legs — especially worse at night
- Cramping pain in the calves when walking that resolves with rest (claudication)
- Feet turning white, then blue, then red with temperature changes (Raynaud's phenomenon)
- Sores or wounds on the feet that are not healing
- One leg significantly colder than the other
Important Causes
Diabetic peripheral neuropathy: The most common cause of foot numbness in India. Chronically elevated blood sugar damages peripheral nerve fibres — the longest nerves in the body (going to the feet) are affected first. Symptoms: stocking-and-glove distribution numbness and tingling, worse at night, burning quality.
Peripheral arterial disease (PAD): Atherosclerotic narrowing of the arteries supplying the legs. Cold, pale, pulseless feet; cramping calf pain with walking. Significant risk factor for heart attack and stroke.
Vitamin B12 deficiency neuropathy: Common in India, especially in vegetarians and metformin users. Causes peripheral neuropathy that mimics diabetic neuropathy — completely treatable with B12 supplementation.
Hypothyroidism: Causes peripheral neuropathy and cold intolerance. Simple blood test (TSH) diagnoses it; easily treated.
