Child Not Talking — When Should You See a Doctor?

No words at 12 months. Not combining words at 2 years. 'Wait and see' is not the answer. Early assessment gives every child the best possible start.

Speech delay is the most common developmental concern that brings Indian parents to a paediatrician. Many parents are reassured to 'wait and see' — but this advice costs children months of precious early intervention time. At Sri Anand Hospital, Dr. Sushma B provides structured speech and language assessment, identifies the specific cause (hearing loss, autism, intellectual disability, late talker), and arranges the right intervention without delay.

Seek Emergency Care Immediately If You Have:

  • 1No babbling at all by 12 months
  • 2Loss of words that were previously used — regression is always a red flag
  • 3Not responding to their name consistently by 10 months
  • 4No pointing, waving, or showing objects by 12 months
  • 5No words at 16 months, or no 2-word combinations by 24 months

Possible Causes of This Symptom

Hearing Loss (Most Important to Rule Out)

Undetected hearing loss is the single most treatable cause of speech delay. If a child cannot hear language clearly, they cannot learn to produce it. Audiological assessment is always the first step.

Autism Spectrum Disorder

Speech delay in autism is accompanied by reduced social communication — limited eye contact, not responding to name, no pointing, reduced interest in social interaction, repetitive behaviours.

Late Talker (Language Developmental Variation)

Some children develop language somewhat later without underlying pathology — 'late bloomers'. Typically have good comprehension, gesture use, and social engagement. Often catch up spontaneously or with brief speech therapy.

Intellectual Disability

Global developmental delay affecting all domains — motor, cognitive, social — of which speech delay is one component. Investigation for cause includes chromosomal and metabolic testing.

Expressive Language Disorder

Child understands language normally but has significant difficulty expressing themselves verbally. Responds to name, follows commands, good comprehension — but limited or unclear speech.

How We Diagnose the Cause

1

Hearing Assessment (First Step)

Pure tone audiometry or ABR (Auditory Brainstem Response) for younger children — confirming or ruling out hearing loss before other assessments.

2

Autism Screening (M-CHAT-R)

Standardised screening questionnaire for children 16–30 months assessing social communication and behaviour — identifying those who need formal autism evaluation.

3

Developmental History and Direct Assessment

Detailed milestone history, observation of child's communication in clinic (eye contact, pointing, play), and parent-completed language samples.
