Possible Causes of This Symptom
Essential Tremor
The most common cause of hand tremor — often mistakenly called 'Parkinson's'. Essential tremor causes shaking during action (writing, holding a cup) and is usually absent at rest. It runs in families and responds well to medication (propranolol or primidone).
Parkinson's Disease
Parkinson's tremor classically occurs at rest — the hand shakes when it is still and on the lap, but less when in use. Usually starts on one side. Accompanied by slowness, rigidity, and small handwriting. Very treatable with medication.
Drug-Induced Tremor
Many common medications cause tremor — antipsychotics, antidepressants, lithium, thyroid hormone, steroids, and even salbutamol inhalers. This is fully reversible once the medication is changed or stopped.
Physiological / Enhanced Physiological Tremor
Fine tremor brought on by anxiety, caffeine, sleep deprivation, thyroid overactivity, or low blood sugar. Worst during stress or exertion. Resolves when the underlying cause is addressed.
Cerebellar Tremor
A tremor that worsens as the hand approaches a target (intention tremor) — caused by cerebellar disease. Associated with unsteady gait and incoordination. Requires brain imaging and cerebellar workup.
How We Diagnose the Cause
Tremor Classification Exam
Assessment of tremor at rest vs. during action, unilateral vs. bilateral, direction (postural, kinetic, intention), and associated features (slowness, rigidity, gait) — all done at the bedside by an experienced neurologist.
Blood Tests
Thyroid function (TSH, T4), blood sugar, liver function, serum copper — to identify systemic causes. Medication review is part of every tremor assessment.
MRI Brain (When Indicated)
Requested when cerebellar or brain pathology is suspected — not needed for typical essential tremor or Parkinson's.
