Cerebral palsy (CP) is the most common cause of childhood physical disability in India, affecting approximately 2–3 per 1,000 live births. There is no cure for CP — but with intensive, consistent physiotherapy starting early, most children make remarkable gains in function, independence, and quality of life.
What Is Cerebral Palsy?
CP is a group of movement and posture disorders caused by damage to the developing brain, usually before or during birth. The brain damage itself does not worsen over time — but without treatment, the secondary effects on muscles, bones, and joints do progress. This is why early, consistent physiotherapy matters so much.
What Physiotherapy Does for CP
Physiotherapy for CP targets multiple goals:
- Improving range of motion: Tight muscles in CP progressively shorten (contractures), limiting movement. Regular stretching and positioning prevents and reduces contractures.
- Strengthening weak muscles: The muscles opposing spastic muscles are often weak. Targeted strengthening improves balance between muscle groups.
- Improving gross motor function: Rolling, sitting, standing, walking — physiotherapy maximises function at each stage of development.
- Preventing deformity: Hip dislocation, scoliosis, and joint deformities are far less common in children who receive consistent physiotherapy.
- Reducing pain: Adults with CP who received adequate childhood physiotherapy have significantly less musculoskeletal pain.
What a CP Physiotherapy Session Looks Like

- Passive and active stretching: 10–15 minutes stretching tight muscle groups (gastrocnemius, hamstrings, hip flexors, adductors — whichever are most involved)
- Strengthening exercises: Adapted to the child's ability level — can be play-based for younger children
- Functional activities: Sitting balance, standing, weight-bearing, gait training — depending on the child's level
- Home programme: Every session includes guidance for parents to continue exercises at home. 10–15 minutes of parent-led exercises daily significantly multiplies the effect of in-clinic sessions
Sessions last 45–60 minutes. For young children, sessions may be shorter (30 minutes) and more frequent.
When Should CP Physiotherapy Start?
As early as possible. The brain has the greatest plasticity (ability to reorganise and compensate) in the first 3 years of life. Physiotherapy during this critical period produces the largest gains. However, physiotherapy at any age produces meaningful improvement — it is never too late to start, and it is never appropriate to stop.
How Often Is Physiotherapy Needed?
Guidelines generally recommend:
- Clinic sessions: 2–3 times per week in early childhood; 1–2 times per week as the child gets older
- Home programme: daily
The home programme is as important as the clinic sessions. A child receiving 2 clinic sessions per week but doing 15 minutes of home exercises daily gets far more total therapy than one who only attends clinic.
