Fever is the most common reason for paediatric emergency visits across India. In most cases, fever in a child is a protective immune response to a viral infection — not dangerous in itself. However, certain patterns of fever, or fever in certain age groups, require urgent paediatric evaluation to rule out bacterial infections, dengue, typhoid, meningitis, or other serious conditions. When Does a Child's Fever Need a Doctor?

Any Fever in a Baby Under 3 Months

A temperature of 38°C or above in a baby under 3 months old is a medical emergency — always seek immediate evaluation. The immune system is immature and serious bacterial infections can progress very rapidly at this age.

High Fever Above 40°C

Very high fever needs clinical assessment to identify the cause and determine whether antibiotics are indicated. Not all high fever needs antibiotics — most is viral — but clinical evaluation is important.

Fever Lasting More Than 3 Days

Most viral fevers resolve within 3 days. Fever persisting beyond 72 hours needs a doctor's assessment — including blood tests to rule out dengue, typhoid, and urinary tract infection.

Fever with a Fit / Convulsion

A febrile convulsion — a seizure triggered by rapid rise in temperature — requires immediate medical evaluation. Although usually benign, meningitis must be ruled out in any child with fever and convulsion.

Very Unwell or Unusual Child

Trust your instinct. A child who is unusually drowsy, not responding normally, not drinking fluids, has a rash, or just 'seems wrong to you' — always comes in, regardless of temperature reading.
