Child Growth Delay — Short Stature and Poor Weight Gain in Hyderabad
Is your child the shortest in the class? Not gaining weight? Growth concerns need specialist paediatric evaluation — many causes are very treatable.
Every parent worries about their child's growth. At Sri Anand Child and Neuro Center, Chanda Nagar, Dr. Sushma B provides comprehensive growth assessment for children with short stature, poor weight gain, or growth faltering. The first step is always plotting height and weight on a standardised growth chart — to determine whether the child is truly outside the normal range or simply small within normal limits. When true growth delay is confirmed, a systematic evaluation identifies the cause — and many causes, including growth hormone deficiency, hypothyroidism, and nutritional deficiencies, are completely treatable.
Key Points
- Short stature is defined as height below the 3rd percentile for age and sex on standard growth charts.
- Most short children have familial short stature (tall families have tall children) — not a medical problem.
- Growth hormone deficiency is diagnosed with a stimulation test — not just a low IGF-1 level.
- Hypothyroidism (underactive thyroid) is a common, treatable cause of growth delay in children.
- Early diagnosis is crucial — growth hormone treatment must be given before the growth plates close.

Signs of Growth Delay to Watch For
Height Below 3rd Percentile
Consistently below the 3rd percentile for age and sex on standardised Indian growth charts. More important than a single measurement: is the child following their growth curve or falling off it?
Poor Weight Gain (Failure to Thrive)
Weight gain that crosses two or more centile lines downward over 4–6 months signals failure to thrive. In infants, inadequate weight gain in the first year can affect brain development.
Bone Age Delay
An X-ray of the left hand (bone age) compares skeletal maturity to chronological age. A delayed bone age suggests a constitutional delay or growth hormone problem; a normal bone age suggests familial short stature.
Signs of Underlying Disease
Chronic diarrhoea, abdominal bloating, pallor, swelling of face and feet, or recurrent infections alongside poor growth point to coeliac disease, kidney disease, or nutritional deficiency as the underlying cause.
Puberty and Height Prediction
Children who mature late (constitutionally delayed puberty) are short for their age but have normal growth potential — they will continue growing after peers have stopped. This pattern is very common and usually familial.
How We Evaluate and Manage Growth Delay
Growth Chart Review and Plotting
Systematic review of all available height and weight measurements, mid-parental height calculation, and plotting on WHO/IAP growth charts to confirm and characterise the growth pattern.
Bone Age X-Ray
Left wrist X-ray interpreted to determine skeletal age versus chronological age — essential for distinguishing constitutional delay from pathological short stature.
Blood Investigations
IGF-1 (growth hormone marker), thyroid function (TSH, T4), haemoglobin, renal function, coeliac screen (anti-tTG), inflammatory markers — based on clinical assessment.
Nutrition Assessment
Detailed dietary history, anthropometric measurements, assessment for micronutrient deficiencies — iron, zinc, B12, vitamin D — and targeted supplementation where needed.
Growth Hormone Therapy (Where Indicated)
For confirmed growth hormone deficiency, daily subcutaneous growth hormone injections are highly effective. Requires specialist endocrine referral — we coordinate and initiate the workup.

"Short stature in a child causes enormous anxiety for parents, often driven by comparisons in class. Most short children are completely normal — they just have short parents. But for the minority with a treatable cause, early diagnosis changes their adult height significantly."
