Every child gets distracted and fidgety sometimes. But for children with ADHD (Attention Deficit Hyperactivity Disorder), difficulty focusing, impulsiveness, and hyperactivity are so significant that they affect learning, relationships, and daily life. ADHD is real, common, and very treatable — but it needs proper assessment.
What is ADHD?
ADHD is a neurodevelopmental disorder — it is about how the brain is wired, not about laziness, bad parenting, or too much screen time. Children with ADHD have difficulty with attention regulation and impulse control. The brain's executive function system — which controls planning, focusing, and self-regulation — works differently.
ADHD affects approximately 5–7% of school-age children globally, with similar rates in India. It is more commonly diagnosed in boys, but girls with ADHD are often missed because they tend to show the inattentive type (daydreaming, disorganised) rather than the hyperactive type (running, shouting, disrupting class).
The Three Types of ADHD
- Predominantly Inattentive (was called ADD): Difficulty focusing, forgets instructions, loses things, daydreams, easily distracted, makes careless mistakes. Often missed — especially in girls.
- Predominantly Hyperactive-Impulsive: Constant movement, cannot stay seated, talks excessively, interrupts, acts before thinking, difficulty waiting.
- Combined (most common): Significant features of both inattention and hyperactivity-impulsivity.
Signs of ADHD at School
- Consistently fails to finish tasks or assignments
- Appears not to listen when spoken to directly
- Frequently loses school materials — books, pens, lunch box
- Avoids tasks that require sustained mental effort (homework)
- Very disorganised — desk, bag, homework diary all chaotic
- Frequently calls out in class, cannot wait their turn
- Very impulsive — acts without thinking, does not consider consequences
- Much better performance in one-on-one situations than in class

How is ADHD Diagnosed in India?
ADHD is diagnosed clinically — there is no blood test or brain scan. Proper diagnosis requires:
- Detailed history from parents — symptoms across home and school, developmental history, family history
- Standardised behaviour rating scales (Conners' Rating Scale or Vanderbilt ADHD Scales) completed by parents and teachers
- Assessment for coexisting conditions — learning disabilities, anxiety, autism, hearing or vision problems
- Exclusion of other causes — thyroid disease, sleep disorders, iron deficiency
What Helps?
The most effective treatment for school-age children combines:
- Behavioural strategies: Structured routines, clear expectations, immediate positive reinforcement, breaking tasks into small steps, scheduled movement breaks
- School accommodations: Preferential seating, extra time for tests, reduced homework load, written instructions — these require a medical letter
- Medication (when indicated): Stimulant medications (methylphenidate) are safe and effective — often producing dramatic improvements in focus and impulse control. They are used when ADHD significantly impairs functioning and behavioural strategies alone are insufficient
- Parent training: Learning how to manage ADHD behaviours at home reduces family stress and improves the parent-child relationship
